This Dr Malone article highlights that HHS has determined that organ donation in the states has been run unethically by some harvesting companies. The thing that jumped out to me was the practice of harvesting from donors when they were still alive.
Many years ago I was warned about this by some family members. They were doctors and leaders of departments in large Toronto hospitals. There advice to me was not to sign or indicate that you were an organ donor because there was a reasonable chance that if you found yourself in the hospital with significant issues, they may decide to harvest your organs rather than treat you.
One big issue was determining when death occurs. You would think the point of death would be clear but it isn't. From their observation they saw donors being declared dead who had flatlined in one area but showed signs of life in others.
I know organ donating can save lives but personally, I would think very carefully about this.
